1, Form 843 Line 1: Period
請問是指2009年的工作期間嗎?(我只有一份工作 很單純 無須seperate)

2, Form 843 Line 3: Type of tax
這邊是想退公司withheld SSN and Medicare 的稅
請問該勾選"Employment" or "Income"?

3, Form 843 Lile 5b: Date(s) of payments
請問這邊是指什麼呢?
